As a reminder, all student health forms for the 2025–2026 school year are due by August 9, 2025.

Alternative Option Available for Student Physicals
- Out of Pocket Physical at Urgent Care or Sports Medicine Facility: If for any reason you are unable to get a physical exam appointment with your Primary Care Provider in order to meet NDP’s due date of August 9, then a student may instead have an "out of pocket" Sports/Pre-Participation School Physical at any Urgent Care or Sports Medicine Facility. Sometimes the Primary Care Provider offers this service as well, so please check with them first.
