Student Health Information and Forms

All students must have a current immunization record and annual physical exam on file in the Health Suite, with the physical exam performed within 12 months of the annual August 9 due date. All required health forms must be submitted by the due date in order for students to begin the school year, participate in any school activities, or try out for a sports team. Alternative (Optional) Opportunities Available for Student Physicals


Out-of-Pocket Exams:
 If for any reason you are unable to get a physical exam appointment with your Primary Care Provider in order to meet NDP’s due date of August 9, then a student may instead have an out-of-pocket Sports/Pre-Participation School Physical at any Urgent Care or Sports Medicine Facility. Sometimes Primary Care Providers offer this service as well, so please check with them first.

Towson Sports Medicine Physical Event: Students also have the option to have a Sports/Pre-Participation School Physical with NDP's team doctor at Towson Sports Medicine. The Towson Sports Medicine Physical Event will take place on Wednesday, June 3, 2026, and is open to all local school students. This is the only date currently being offered. Please print all NDP Health forms required and bring them with you to this appointment.

Absence Due to Illness Policy

To help ensure the health and well-being of all students, faculty, and staff, Notre Dame Prep follows a clearly defined Absence Due to Illness Policy, developed in collaboration with our Health Suite. This policy, as outlined in the Student and Employee Handbooks, provides important guidance for when a student is sick, including when to stay home, how to report an illness, and when a medical note is required. By following these protocols, we aim to reduce the spread of illness on campus and support students in their recovery.
