As a reminder, our electronic Student Handbook Acknowledgement Form & Media Release Form are due by August 29, 2025, in PrepNet. You may access and electronically sign both forms starting on August 22, 2025, under your Files & Forms tab in PrepNet. Please review additional details about both forms below:

• Media Release Form: The Media Release Form for the 2025–2026 school year grants or denies Notre Dame Preparatory School permission to include your student in school photography, videography, school social media accounts, digital and print publications, and on the school's website. If you have any questions about this form, please email the Communications Office.
  
  PLEASE NOTE: This form requires an electronic signature by only ONE parent/guardian per student. If the form is completed by one parent/guardian, the form will appear as "completed" under the other parent/guardian's Files & Forms tab on their PrepNet profile.
  
  
• Student Handbook Acknowledgement Form: All students and families are required to review and sign the Student Handbook Acknowledgement Form, which acknowledges that you and your student have reviewed the 2025–2026 Student Handbook. A digital copy of the 2025–2026 Student Handbook can be accessed here and also under the Resources section of PrepNet. All students will receive a hard copy of the 2025–2026 Student Handbook and planner during the first day of school on Monday, August 25, 2025.
